In this multicenter, international randomized trial, a liberal transfusion strategy did not significantly reduce the risk of unfavorable outcomes in ICU patients with a moderate or severe TBI at 6 months compared to a restrictive transfusion strategy. However, a liberal transfusion strategy was associated with better scores on some quality of life measures. A restrictive transfusion strategy for managing anemia in patients with a TBI may be appropriate, but further research is needed to assess the impact on nuanced measures of neurological recovery.
